We’re collecting real experiences — good, bad and everything in between — to identify patterns in what is working, what isn’t, and where people are struggling to get the help they should be able to access.

We only ask for your general area so we can understand whether people’s experiences differ across the country. We do not ask for your address or postcode.

Choose the type of support that best matches your experience. You can add another service if your experience involved more than one.
3. What was your experience?
Tell us what happened in your own words. This could be about finding the service, asking for help, waiting, being assessed, the support you received, being refused help, what happened afterwards, or anything else that mattered to you.
Please don’t include your name, address, case or reference numbers, names of individual staff members, or other details that could identify you or somebody else.

Your experience will be stored anonymously. We may analyse experiences together to identify patterns in how services are working.
We may also publish anonymous experiences or extracts from them. Submissions will be reviewed before anything is published so that identifying information can be removed.
